目的:观察头穴围刺结合运动疗法对脑梗死大鼠血管新生的影响。方法:采用健康雄性wistar大鼠75只随机分为假手术组、模型组、头穴围刺组、运动组、围刺+运动组,每组15只。参照Zea-Longa报道的线栓法,制备大脑中动脉梗死(MCAO)脑缺血再灌注模型,采用对应的方法进行干预,干预14天后进行行为学评估;每组随机选取5只大鼠用TTC染色法测定脑梗死面积比;RT-PCR法检测β-catenin mRNA、GSK-3βmRNA表达水平,western-blot检测血管内皮细胞VEGF水平。结果:14天后围刺+运动组mNSS评分、脑梗死面积比优于模型组、头穴围刺组和运动组(P<0.01)。围刺+运动组与其它各组相比可明显上调β-catenin蛋白表达、下调GSK-3β水平、增加VEGF表达(P<0.01)。结论:头穴围刺结合运动疗法可促进脑梗死大鼠的血管新生。  相似文献

目的:探讨舌三针联合康复训练对脑梗死后假性延髓麻痹所致构音障碍疗效,以期提高脑卒中后患者生命质量。方法:选取2016年12月至2019年1月海南省人民医院收治的脑梗死后假性延髓麻痹所致构音障碍患者78例进行回顾性分析,按照治疗方法不同分为对照组和观察组,每组39例。对照组常规西医+康复训练治疗,观察组在以上基础上加用舌三针治疗,均连续治疗6周。观察2组治疗前、完成治疗后血脂、Frenchay构音评分、血液流变学、言语空气动力学指标变化并比较。完成治疗后总结疗效情况。结果:1)2组治疗完成后反射、呼吸、唇、颌、软腭、喉、舌、言语评分和治疗前比较均显著上升(P<0.05),且观察组上升幅度显著高于对照组(P<0.05)。2)2组治疗完成后三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白(LDL-C)较治疗前均显著下降,高密度脂蛋白胆固醇(HDL-C)较治疗前比较均显著上升(P<0.05),且观察组上升幅度显著高于对照组(P<0.05)。3)2组治疗完成后全血高低切黏度、血浆黏度、红细胞压积较治疗前均显著下降(P<0.05),且观察组优于对照组(P<0.05)。4)对照组治疗前、完成治疗后各指标比较差异无统计学意义(P>0.05),观察组治疗完成后平均声压级(MSDV)、音调(P)、最长发声时间(MPT)、平均气流率(MFR)、声门下压(SGP)、声门阻力(GR)、发声效率(VE)较治疗前均显著升高(P<0.05),且观察组优于对照组(P<0.05)。5)观察组治愈率38.46%、总有效率89.74%,显著高于对照组治愈率23.08%、总有效率79.49%(P<0.05)。结论:舌三针联合康复训练能通过促进脑微循环,改善血脂,促进言语功能来改善脑梗死后假性延髓麻痹所致构音障碍症状,提高生命质量。  相似文献

BackgroundCerebral venous sinus thrombosis (CVST) is a rare complication of pregnancy. It usually presents with a headache and may mimic a post-dural puncture headache (PDPH) in women who receive a neuraxial block.MethodsMedline, CINAHL and EMBASE databases were searched to identify postpartum cases of CVST following neuraxial block. The aim was to delineate the characteristics, presentation, investigations, and outcomes of postpartum women who presented with CVST.ResultsForty-nine articles with 58 case reports were identified. Forty-two women (72.4%) had an epidural attempted whilst 16 women (27.6%) received a spinal anaesthetic. Accidental dural puncture (ADP) was reported in 17 women (40.5%). Headache was the presenting symptom in 57 women (98.3%); 26 women (44.8%) also experienced seizures. Post-dural puncture headache was reported in 46 (79.3%) and an epidural blood patch was performed in 26 women (44.8%). Superior sagittal sinus, transverse sinus, and cortical veins were the most common sites of thrombosis. The median time to diagnosis was 6.5 days from delivery. Magnetic resonance imaging was the most common diagnostic neuro-imaging modality. Full neurological recovery was reported in 48 (82.8%), whilst neurological deficits were reported in six (10.3%) women.DiscussionThe diagnosis of CVST may be overlooked in women who present with headache following neuraxial block. A change in character of headache with loss of postural element, and focal neurological signs are the key clinical features that could help anaesthetists differentiate headache of CVST from PDPH. The high reported rates of ADP and PDPH lend support to their possible association with CVST.  相似文献

Gravity has captured the attention of thinkers since Aristote. Scientists are always trying to more understand the physical mechanisms and their impacts on terrestrial and extraterrestrial life. Despite all the progress in scientific and clinical research, we still don’t know certain mechanisms of functions and the extraordinary powers of this gravity. Moreover, man is one of the living beings that is directly linked to this gravity. Scientific studies prove the impact of this gravity on height and weight development, the functions of vital organs due to inactivity, and the socio-cultural behavior of human beings. From the neurophysiological point of view, the biomechanics of man in connection with the sensory-motor vectors are intimately linked to the vector of the vertical. This vector is in perpetual change during the passage of the man from the lying position to the standing one and obviously during the walk-in order to build a trajectory and a direction to the movement. However, little scientific and clinical evidence has been revealed on the effect of gravity in the field of Cerebral Palsy and in particular that of multiple disabilities. The complex and heterogeneous clinical picture raises the limits of research in the field of multiple disabilities. However, international consensus and studies are gradually emerging trying to better understand the importance of this severity on the health of people with multiple disabilities. This case study of a child with multiple disabilities highlights the news in the literature review. The Physiotherapist evokes this clinical and scientific problem of the device and its link with gravity by proposing an installation which he calls “MOBIVERT” to put the child in charge in a course of listening, observation and sharing. of experience with the medical, paramedical, psycho-educational and social fields.  相似文献

BackgroundThe collagen type IV alpha 1 chain (COL4A1) is an essential component of the basement membrane in small vessels. Pathogenic variants in COL4A1 cause perinatal cerebral hemorrhages in an autosomal-dominant fashion. However, little is known about the long-term outcomes of patients with mildly affecting COL4A1 mutations.Case reportWe report a 17-year-old boy, who presented with recurrent intracranial hemorrhages in the periventricular white matter. He had been followed-up as a child with cerebral palsy bearing intracranial calcifications, developmental delay and epilepsy. Screening tests in infancy provided negative results for intrauterine infections. Severe motor and cognitive deficits persisted after admission. Carbazochrome was introduced on day 19 of admission, which appeared to prevent extension and reactivation of cerebral hemorrhages for over 6 months after discharge.ResultsTargeted sequencing of NOTCH3 and TREX1 excluded causal mutations in these genes. The whole-exome sequencing revealed that he carried a de novo mutation in COL4A1 (p.Gly696Ser). An overview of the literature for 345 cases with COL4A1 mutations supported evidence that p.Gly696Ser is associated with the unique phenotype of late-onset hemorrhage among patients with COL4A1-associated cerebral angiopathy.ConclusionsThis case first demonstrates that infants with COL4A1-associated leukoencephalopathy and calcifications have a risk for developing the rupture of small vessels in the cerebral white matter after 10 years of age.  相似文献

Children with severe central neurological disorder (cerebral palsy, the multihandicapped) suffer from impaired coordination of their respiratory muscles. The relative immobility of these patients, the chest deformities and the risk of epilepsy and gastroesophageal reflux can lead to disorders of ventilation/perfusion and to difficult swallowing. These disorders may in turn cause atelectasis and choking episodes favoring the occurrence of respiratory infections. In these patients, recurrent chest infection is the first cause of death. Instrumental airway clearance techniques adapted to these patients are reviewed in this report. Techniques such as the Cough-Assist®, Intrapulmonary Percussive Ventilation (IPV®), high-frequency chest wall oscillations (type VEST®), PEP mask or devices delivering intermittent positive pressure breathing (IPPB) can be used in combination with manual techniques or when the latter become insufficient. In practice their application is challenging, but it is worth trying them individually in each patient to assess their effectiveness.  相似文献

目的:对应用低分子肝素钙与奥扎格雷联合对患有脑血栓疾病的患者实施治疗的临床效果进行研究。方法整群选择在该院2012年12月—2014年12月就诊的患有脑血栓疾病的患者86例,随机分为对照组和治疗组,每组43例。采用奥扎格雷对对照组患者实施治疗;采用低分子肝素钙与奥扎格雷联合对治疗组患者实施治疗。对比神经功能缺损评分在药物治疗前后的变化幅度、脑神经功能恢复正常时间和脑血栓药物治疗计划实施总时间、脑血栓疾病药物治疗效果、用药期间的不良反应人数。结果治疗组患者神经功能缺损评分在药物治疗前后的变化幅度明显大于对照组;脑神经功能恢复正常时间(9.66±2.41)d和脑血栓药物治疗计划实施总时间(13.28±2.14)d明显短于对照组(13.62±3.47)、(17.39±3.20)d;脑血栓疾病药物治疗效果(总有效率90.6%)明显优于对照组(总有效率69.8%);用药期间的不良反应人数(1例)明显少于对照组(8例)。结论应用低分子肝素钙与奥扎格雷联合对患有脑血栓疾病的患者实施治疗的临床效果非常明显。  相似文献

目的:探讨糖尿病对脑梗死出血转化及各分型发生率的影响,为临床制定诊疗方案及预后评估提供有价值的资料。方法回顾性分析运城市中心医院及平陆县人民医院神经内科2011年1月—2015年6月间收治的495例临床资料完整的脑梗死住院患者,根据是否有糖尿病将所入选患者分为糖尿病组及非糖尿病组,通过查阅患者影像及临床资料,观察2组住院期间脑梗死出血转化发生率及不同分型出血转化发生率情况。结果糖尿病组153例中发生出血转化76例(49.7%),其中脑实质血肿型21例(13.8%);非糖尿病组342例中发生出血转化113例(33.0%),其中脑实质血肿型17例(5.0%),2组比较差异具有统计学意义(P<0.01)。结论糖尿病能增加脑梗死出血转化的发生,并能增加更具临床意义的脑实质血肿型出血转化的发生。  相似文献

The feasibility to measure brain perfusion using intravoxel incoherent motion (IVIM) MRI has been reported recently with currently clinically available technology. The method is intrinsically local and quantitative, but is contaminated by partial volume effects with cerebrospinal fluid (CSF). Signal from CSF can be suppressed by a 180° inversion recovery (180°‐IR) magnetization preparation, but this also leads to strong suppression of blood and brain tissue signal. Here, we take advantage of the different T2 relaxations of blood and brain relative to CSF, and implement a T2‐prepared IVIM (T2prep IVIM) inversion recovery acquisition, which permits a recovery of between 43% and 57% of arterial and venous blood magnetization at excitation time compared with the theoretical recovery of between 27% and 30% with a standard 180°‐IR. We acquired standard IVIM (IVIM), T2prep IVIM and dynamic susceptibility contrast (DSC) images at 3 T using a 32‐multichannel receiver head coil in eight patients with known large high‐grade brain tumors. We compared the contrast and contrast‐to‐noise ratio obtained in the corresponding cerebral blood volume images quantitatively, as well as subjectively by two neuroradiologists. Our findings suggest that quantitative cerebral blood volume contrast and contrast‐to‐noise ratio, as well as subjective lesion detection, contrast quality and diagnostic confidence, are increased with T2prep IVIM relative to IVIM and DSC.
